The Triangle of Care – Best Practice in Dementia Care

The Triangle of Care initiative for dementia, developed in partnership with the Royal College of Nursing, as well as people with dementia and their carers, sets out a framework for meaningful engagement between professionals and those with first-hand experience of the condition.

It builds on a similar initiative for mental health services by the Carers Trust in partnership with Royal College of Nursing to adapt the Triangle of Care to meet the needs of carers of people with dementia when that person is admitted to a general hospital.

This guide identifies six key standards required to achieve better collaboration and partnership with carers. For each standard we suggest good practice examples and resources that may be helpful.

In order to ensure these six key standards of carer engagement exist and remain in place, a self-assessment audit tool for carer engagement is also included.
