SCIE’s (Social Care Institute for Excellence) Integration Tool
is for interdisciplinary teams is intended as an evaluation tool, to develop
effective integrated working practices and to address challenges. It’s on
online working module, where teams can work individually or together through
the different modules. It gives teams a structure on how to approach
integrated care and to record their responses, agreements and plans.
It includes four exercise modules: Organizational issues;
cultural and professional issues; contextual issues; and co-production
issues.
