The National End of Life Care Program
and its partners have created an
interactive toolkit to guide healthcare professionals in creating
personalized, integrative care at the end-of-life. The toolkit identifies
the four key components of commissioning any end-of-life program: analyze and
plan; design pathways; specify and procure; deliver and improve. It calls
for an integrated approach that includes health care professionals from a
variety of departments, including nurses, physicians, social workers and
spiritual care services. It also includes examples of what well-commissioned
end-of-life care should look like.
