Commissioning Toolkit: A toolkit for health and social care professionals

The National End of Life Care Program and its partners have created an interactive toolkit to guide healthcare professionals in creating personalized, integrative care at the end-of-life.  The toolkit identifies the four key components of commissioning any end-of-life program: analyze and plan; design pathways; specify and procure; deliver and improve.  It calls for an integrated approach that includes health care professionals from a variety of departments, including nurses, physicians, social workers and spiritual care services. It also includes examples of what well-commissioned end-of-life care should look like.
