The Role of an Intermediate Unit in a Clinical Pathway

This study explores an intermediate unit's role in a clinical pathway for older patients in transition between hospital and home.  The intermediate unit studied is a collaboration between a University Hospital and four municipalities in Norway. Qualitative data were collected from patients and healthcare providers.
