patients who have complex health needs and require care from a variety of
healthcare professionals in a patient-centered medical home.
Coordinating this type of care is often challenging, especially for small
institutions. The paper focuses on 5 programs that have addressed these
challenges in Patient-centered medical homes, and ways in which small
facilities can deliver this type of care. Chapters include: barriers to
