Interventions to Reduce Acute Care Transfers

INTERACT II (Interventions to Reduce Acute Care Transfers) is designed to improve the early identification, assessment, documentation, and communication about changes in the status of residents in skilled nursing facilities. The goal of INTERACT II is to improve care and reduce the frequency of potentially avoidable transfers to the acute hospital.

The INTERACT II Tools, educational materials, and implementation strategies were developed by Drs. Joseph Ouslander, Gerri Lamb, Alice Bonner, and Ruth Tappen, and Laurie Herndon with input from a variety of direct care providers and national experts in a project supported by the Commonwealth Fund based at Florida Atlantic University.

The INTERACT II tools provided are: 1) Communication tools; 2) Care Paths or Clinical tools; and 3) Advance Care Planning tools.
