Oral Hygiene Pocket Docket Smart Care Cards

Pocket Dockets are Smart Care Cards for personal support workers caring for older adults to:

• be knowledge-informed,
• review up-to-date care techniques, and
• assist with client-centred care planning.

The information on the care card is organized under 4 headings: i Know, i See, i Tell and i Do.

More cards will be available in the near future on sleep, stroke, hypertension and more.
