This report presents the finding of a scoping review conducted by the Canadian Research Network for Care in the Community exploring 47 modles of integrating health and social care for older persons.
It also aims to shed light on innovations and successful best practice intitiatives underway across Canada and internationally. The review indentifies fundamental design dimensions of intregrating initiatives and key transferable lessons to help inform policy and practice, and provide guidance to on-going aging at home efforts across jurisdictions.
