Your health, your way, developed by the NHS Social Care Partnership UK, provides people with long term conditions with information about the choices which should be available to them locally, to self care in partnership with health and social care professionals.
To coincide with the issue of a patient information leaflet on living life well with a long term condition, the NHS have developed a corresponding information booklet and resource pack for health professionals.
The Information booklet will enable healthcare professionals to effectively increase their own and patients’ understanding and awareness of self care, and promote shared decision making to empower patients and their carers in decisions about their care. The on-line Resource Pack will support them to provide local and national information about self care services together with practical resources, tools and signposting.
