Trigger Tools have been developed by the Institute for Healthcare Improvement to prioritize a list of triggers that can be used to detect adverse drug events in a variety of care settings, including the hospital and ambulatory care.
This tool was developed by the University of Pittsburgh, Division of Geriatric Medicine and Department of Biomedical Informatics to monitor adverse drug effects.
Adverse drug events (ADEs) are a common cause of morbidity and mortality in the nursing home setting. Despite their frequency and potential impact, current methods of ADE detection leads to a significant number of undetected events.
The Trigger Tool for Measuring Adverse Drug Events in the Nursing Home was developed by a multidisciplinary expert panel. The panel reached consensus agreement on a defined list of signals of potential ADEs in nursing home residents. This is a necessary initial step toward detecting and reducing the future occurrence and effect of ADEs in the nursing home setting. The results of this study can be used to prioritize an initial list of signals to be included in paper- or computer-based methods of potential ADE detection.
The tool is downloadable at no cost, but you must register and sign in to download.
