From the Health Care Innovations Exchange, a new AHRQ (Agency for Health Care Research and Quality, U.S.) program, designed to support health care professionals in sharing and adopting innovations that improve the delivery of care to patients. The site contains innovative strategies and quality-related tools, designed to improve your organization's ability to innovate and adopt new ideas, and interact with innovators and adopters.
Featured on the inaugural site is a project with the goal of preserving health and helping elders remain in their homes with dignity as long as possible.
An interdisciplinary team of geriatricians, nurse practitioners, and social workers provide primary health care through house calls to homebound, frail elders. By identifying and treating problems before they become acute, the House Call team has successfully reduced use of emergency care, hospitalizations, and nursing home placements among the target population and has achieved shorter length of stay when hospitalizations do occur.
