Published by the Royal College of Nursing in May 2004, this toolkit was developed to “assist nurses in the identification and articulation of their contribution to the health and social wellbeing of older people” through helping to determine levels of nursing intervention and the input from registered nurses required, as well as contributing to the care planning process.
Primarily intended for registered nurses undertaking an assessment of an older person in a care home, the toolkit can also be used by nurses in hospitals and in the community, such as in contributing to the Single Assessment Process (SAP). The publication discusses the role and purposes of assessment and the role of nurses in meeting continuing care needs.
It describes the five stages of the RCN assessment tool along with their rationale and the scoring systems used to establish levels of nursing intervention required, and the input needed for different elements of interventions. This in turn can be used for workforce planning and to help in establishing appropriate skill mix.
