Quality
of Life Outcomes for People with Alzheimer’s Disease and Related Dementia
is a guide that provides good practice examples and measurable indicators to
support quality of life. Written by the Wisconsin Statewide Advisory
Committee on Quality of Life Outcomes for People with Alzheimer’s Disease and
Related Dementia, the tool is meant to be used by care providers, caregivers
and persons with cognitive impairment as a way to plan for a good quality of
life as the disease progresses.
There are five sections: physical well-being; meaningful
relationships; preparing for the future; individuality and staying active in
life activities. Within each of these sections are observable indicators
and examples of good practice. Persons with cognitive impairment can go through
the list with care providers and caregivers before the disease progresses to
indicate with factors are most important to them. Care providers and
caregivers can also go through the list on the persons’ behalf.
The guide also includes procedures on how to use the tool and
suggested resources for further information.
